Membership Checklist

Welcome to the membership paperwork page.
If you are a returning chapter advisor at the same chapter as last year, you can start on this page.

If you are a new advisor or an advisor at a new chapter, please refer to the following links for more information:

Collect Membership Information

  1. You will need at least 5 paid student members and an advisor to register a new chapter or renew
  2. You will need the following information for each student: First Name, Last Name, Gender, Race, Grade, Email and Phone Number (optional)

  3. Check for spelling errors and duplicate members
  4. Please be VERY cautious not to duplicate entries. Once members have been added to the system they cannot be removed, even by the National office.

Enter Data into Management System

  1. You will need your chapter number and password. If you forgot your chapter number and/or password, please contact the Connecticut HOSA State Advisor at stateadvisor@cthosa.org.
  2. Please ensure that all membership dues have been collected by your school administrator
  3. After all members have been added, please wait for invoice to be generated. This could take up to a week
  4. Submit payment within 30 days of invoice being created
  5. Please mail payment to HOSA International, 548 Silicon Drive, Suite 101, Southlake, TX 76092 (include the invoice and your chapter number with check)
